How to make it

  1. In Google Forms, click Send, choose the link icon, and copy the link (tick "Shorten URL" if you like).
  2. Make sure the form accepts responses from anyone, unless people must sign in with your organization's account.

The form is set to accept responses only from your organization, or to collect verified emails. Change it in the form's Settings > Responses if anyone should be able to answer.
